Application for sickness benefit

  • Submit digitally
    The fastest and easiest way is to submit the application digitally. You will be asked to log in.

  • Take a photograph and upload
    You take a photo of sick note Part D, which your doctor has filled in. Log in and upload the sick note.

  • Submit by post
    You print out a cover sheet which you send in the post together with sick note part D. You get the sick note from your doctor. You do not need to log in.
