Below are various application forms and a list of the documents you need to enclose when you submit an application to NAV.

Forms
- Applications for maintenance support (custodial parent) - NAV 54-00.05
- Applications for maintenance support (non-custodial parent) - NAV 54-00.08
- Additional information (support) - NAV 54-00.15. This form must be completed by both parties.
Documents to be submitted with your application
- Personal income (gross income) and tax return information from both parents (custodial and non-custodial).
- Private written access agreement. If you do not have a written agreement concerning access, please describe your access arrangements.
- Official decision concerning access (country governor or court ruling).
- Dual domicile agreement pursuant to section 36 of the Children Act.
- Custodial parent only: confirmation of child care costs (if you receive support for child care costs).
- Custodial parent only: actual costs (amount per month) of child care if you do not receive support for child care costs.
- In connection with applications for 18 year support (18-årsbidrag): confirmation of attendance at upper secondary school or apprenticeship contract
- In connection with applications for extraordinary child support (særtilskudd), the following must also be enclosed: documentation of expenses and, as applicable, documentation that the expenses are
necessary
