Below are various application forms and a list of the documents you need to enclose when you submit an application to NAV.

Child benefit is generally paid automatically for children born in Norway. In other words, parents do not need to fill in
a form. Below is a list of instances in which child benefit is not paid automatically and an application must be submitted.
Forms
- Child benefit application form - NAV 33-00.08
- Supplementary form for claims for child benefit and/or cash-for-care benefit on the basis of the rules regarding export pursuant
to the EEA Agreement - NAV 34-00.15
Documents to be submitted with your application
- Confirmation of residence permit if you or the child is not Norwegian or a citizen of a Nordic country.
- Documentation indicating when care was transferred. This applies only in cases where the applicant has not received adoption
allowance or a lump-sum grant in connection with adoption.
- Birth certificate for children not registered in the Norwegian National Population Register.
Applications for extended child benefit shall have the following documents enclosed:
- Divorce certificate or separation order (proof that you are separated). NB: only if your civil status is not registered with
NAV
- Mediation certificate for cohabitants with joint children under the age of 16 years
- Declaration of break-up of relationship: for cohabitants with joint children over the age of 16 years and for married couples
who have been split up for at least six months (de facto separation).
- Dual domicile agreement: In connection with applications for split payment. The agreement must be signed by both parents.
- Documentation of imprisonment or similar (for prison sentences or remand in custody lasting longer than six months).
Documents that must be enclosed if you are applying for child benefit pursuant to the EEA rules or Nordic convention:
- Employment contract
- Birth certificate of the children for whom child benefit is being applied for, if the children are not registered in the Norwegian
National Population Register
- Copy of your tax card, as proof of your Norwegian national identity number or D number
- As applicable, the EEA forms E 401, E 411 and E 101 from another EEA member state. This applies to people who have brought
one or more of these forms with them from their home country. The forms E 401 and E 411, which NAV always obtains itself,
need to be renewed each year. E 101 is a certificate confirming that you are an overseas employee posted to Norway, and generally
means that you do not have social security rights in Norway.
